The magnitude of public health impact of the metabolic syndrome is reflected by an estimated prevalence of approximately 47 per cent in adults in the United States (1) to 11.2 per cent in a study from Chennai, India (2). Cardiovascular mortality and all-cause mortality are increased in men with the metabolic syndrome, with women having a heightened risk of coronary disease. The connection between severe mental illness and the metabolic syndrome is emerging as a public health issue of importance to both mental health and primary care practitioners. Despite the risks, many patients with serious mental illness (SMI) have limited access to general healthcare, with less opportunity for cardiovascular risk screening and prevention than would be expected in a non-psychiatric population. World Health Organization (3) and the National Cholesterol Education Program (NCEP) Adult Treatment Panel (4) provided some of the early working criteria for the diagnosis of metabolic syndrome. Using the latter criteria, Heiskanen et al (5) found that the frequency of metabolic syndrome was 2-4 times higher in a group of people with schizophrenia, treated with both atypical and typical neuroleptics, than in an appropriate reference population. A more recent definition, by the International Diabetes Federation6 stressed the importance of waist circumference, using both more stringent and ethnic/ race specific criteria.
